To submit a project for approval, you are required to submit an application for Ethics and a separate application for Research Governance. Click on the below links for the required documentation.

This includes the SSA Form, the Budget, the Clinical Trial Research Agreement, Insurance and Indemnity, Site-specific departmental approval forms, etc.
