Service Coordination Tool Templates (SCTT) General practitioners can refer using the Victorian State Wide Referral Form (VSRF) incorporated into most clinical software. |
Eligibility
HARP CDM provides services to people who have complex needs and/or significant complications as a result of chronic disease including, but not limited to, heart disease, chronic respiratory disease and complex diabetes who:
- have presented to or been admitted to The Royal Melbourne Hospital or other nearby hospital (not mandatory for diabetes services)
- are at imminent risk of hospitalisation
- live in the catchment area (Moreland, Moonee Valley or Melbourne LGAs or Broadmeadows).
Some service components of the program have additional eligibility to ensure care is targeted at those in most need.
Catchment area
The catchment area includes clients who reside within the following areas:
- City of Melbourne
- City of Moonee Valley
- City of Moreland
- Broadmeadows
It is recommended that clients residing in other areas are referred to their local HARP CDM Program. contact details for other HARP programs can be obtained from the Department of Human Services.
