How is an appointment made?

Doctors wishing to make a referral can print and send the form. People concerned about their own family history can call us directly to discuss an appointment. Once we receive a referral, a questionnaire is posted for collection of the individual's family history prior to the appointment.

Before an appointment

A questionnaire is sent to the person attending the appointment. The questionnaire is used to collect information about the family history of cancer. From this information, a family tree is drawn up and used at the appointment. This helps us give the most accurate picture of the person's situation possible and saves time at the appointment.

To arrange a referral

Please ask your doctor to print and fill out the referral form and fax or post to:
