Service Description

The falls and balance clinic provides multidisciplinary assessment and a management plan for clients with falls, mobility and balance problems, aiming to: improve client’s mobility; reduce client’s falls risk; maintain/optimise client’s function and; reduce carer burden. It is primarily an assessment service, with referral to community services and liason with the GP to implement the recommended strategies.

Clients will be prioritised according to referral information

Routine referrals

  • General eligibility
  • Clients who have falls as their main problem and no obvious explanation for the falls
  • Unsteadiness/loss of confidence resulting in significant avoidance of activity
  • Community dwelling or low level care resident
  • Needs medical and allied health assessment
  • Dizziness/Vestibular problems with associated falls

Referral Process

All referrals must include:

  • Name, age, date of birth
  • Address
  • Contact telephone numbers
  • Medicare number
  • Specific clinical problems requiring consultation, including number and recency of any falls
  • Preferred language. Please indicate if interpreter is needed
  • Copy of results of relevant investigations performed within the previous 12 months
  • List of current medications
