Information for Clients and Referrers

The RMH Chronic Wound Service is a multi-disciplinary consultative service for people with chronic wound management needs.

Any patient who has had a wound for greater than four
weeks, or has a history of recurring wounds, is eligible.
The Chronic Wound Service has a team of health professionals with expert knowledge and experience in wound management.

The service consists of Clinical Nurse Consultants in Wound Management, a podiatrist specialising in the high-risk foot, a medical practitioner, a social worker and a dietitian.

How we can help
The Chronic Wound Service can provide assistance with the management of patients who require compression, wound debridement, offloading, dressing selection and wound related nutritional support.

We provide education to patients and their carers to promote healing and skin health.

We can arrange referrals to community health providers for
those patients requiring support services.

Catchment
The Chronic Wound Service geographical catchment includes the cities of Melbourne, Moreland and Hume, but can accept out of area catchments after discussion.

Initial assessment
The initial assessment usually takes about 90 minutes. At this time we aim to determine the likely cause of the wound and devise a management plan in collaboration with the patient.

Ongoing review
Most patients need to return to the clinic weekly or fortnightly at the beginning of their treatment. This is to ensure that the new treatment can be evaluated. Once a treatment is deemed effective for the patient then follow-up review will be less frequent.

Communication
The Chronic Wound Service will fax a letter to the treating GP after the initial assessment and if there are any changes to the patient’s ongoing management. The Chronic Wound Service will send a copy of the GP letter to other community nursing or allied health teams if indicated.

Enquiries and referrals are welcome from doctors, other health service providers, people with wound healing problems, their family members or carers.

The Direct Access Unit staff will determine suitability and urgency, with reference to the Chronic Wound Service team when indicated.

for outreach to residential aged care facilities

Thursdays   1pm-4pm
for patients recently discharged from RMH City Campus or RMH Royal Park Campus

Interpreter Service
An interpreter can be arranged for those patients whose first language is not English, and this is encouraged, particularly for the first assessment.

Cost
There is no charge for the Chronic Wound Service, but materials used outside the clinic are not supplied.
