HARP is an interim service, developing a comprehensive care needs assessment and care plan with the client and family. This often involves linking with other community services, liaising with other family members, health care professionals and GPs. Clients are allocated a key worker within the service area most appropriate to their needs.

Target population
The target population for HARP are people who are most likely to benefit from integrated care and have the potential to reduce avoidable hospital use.
These are people who frequently use hospitals or are at risk of hospitalisation and include:

People with chronic heart disease;

People with chronic respiratory disease;
-
Older people with complex needs; and
-
People with complex psychosocial needs;
-
People with complications as a result of diabetes.

Services are provided to clients who reside within our catchment area of:
City of Moreland
City of Melbourne
City of Moonee Valley
Clients who do not reside within these geograpical boundaries may be referred to other HARP services within their local area (see links).
Funding and client fees
The Department of Human Services provides funding for the HARP program. Most services are without cost to the client.
Nominal charges apply for the following services:

Respiratory rehabilitation groups – $3 per session

Respiratory maintenance programs - $3 per session

Falls Prevention – Strength and Balance program – gold coin donation

Diabetes Foot Program - Community health service fees apply - from $8.00 per session depending on client income.
Client consent and privacy
People who are referred to the service during a hospital visit will be asked to give their verbal consent.
At first contact with the program service, the staff member will explain the service and obtain formal permission to share client's information between the PIH partners involved in their care in line with Health Privacy Legislation (see consumer information).
