RMH Departmental Requirements

To obtain your Statements of Approval, the service departments listed below also require you to complete their internal departmental forms.

• Health Information Services/Medical Records - (complete these forms where Medical Records Services are required)
• Pathology - (complete form where Hospital Pathology services are required)
• Pharmacy
• Radiology

To ensure you are using the latest version of the application form, please download the document directly from the Office for Research website each time you prepare a new application.
