The SLA is signed by at the executive level. Input from all WH staff members is welcomed.

Requesting a service change
Requests for service changes, additional services and new tests should be directed to the WH Pathology Committee. All requests/applications will be reviewed by the Pathology Committee and are subject to approval by executive. Service changes will be added to the SLA. |
