| Primary Care Psychiatry Program (PCP) |

The Primary Care Psychiatry Program (PCP) (formerly CLIPP – Consultation and Liaison in Primary-care Psychiatry) is part of the Shared Care Team at North West Area Mental Health Service (NWAMHS).
In partnership with over 30 GPs, PCP provides care to approximately 120 consumers who were previously managed by NWAMHS’s continuing care teams.
The PCP team manage each consumer’s transition from case management at NWAMHS into primary care by GPs. The PCP Coordinator facilitates this share-care model by supporting the needs of consumers, carers and GPs.
The Program supports GPs and their practices through an integrated education program, which includes formal training, a GP small learning group and the establishment of a mental health nurse in the general practice network. The GPs receive clinical input and training from the PCP team at regular intervals. This ensures a collaborative approach to the provision of mental health care, and better health outcomes for consumers.
