Aged Care Assessment Service Case Management Services
Aged Care Assessment Service (ACAS)
The North West ACAS provides assessment, information and advice to clients and their carers to help them make informed decisions regarding individual care needs. The service supports people in their own homes and communities by linking them to health services, which promote, as far as possible, rehabilitation and restoration of function. The ACAS can also refer people to community services which will maximise their independence and safety. The ACAS has Commonwealth Government delegation responsibility to assess eligibility for nursing homes, hostels, Community Aged Care Packages, EACH (Extended Aged Care at Home), EACH Dementia and TCP (Transition Care) packages if required.

Case Management Services
Case Management Services has responsibility for the provision of case management and in-home support services via Commonwealth funded:
- Community Aged Care Packages (CACPs)
- Extended Aged Care at Home (EACH) program
- Extended Aged Care at Home Dementia (EACH Dementia) program
All programs work primarily with frail aged clients, many of whom have complex medical issues and/or cognitive impairment, in their own homes and utilize a care co-ordinator (case management) model of care.
Eligibility
In order to determine eligibility, clients must be referred to North West ACAS for a comprehensive assessment. Clients must be assessed as requiring:
- Low level residential care for CACPS
- High level residential care for EACH & EACH Dementia
Services
The packages can provide a range of services including:
- personal care
- home care
- nursing
- basic gardening
- in-home respite
- transport and escort to appointments
Services are delivered through a mix of directly employed carers and brokered agency staff.
Community Aged Care Packages (CACPS)
Community Support Services manage a total of 120 CACPs within the Local Government areas (LGA) of Hume, Moreland, Darebin and Yarra.
The packages are designed for people who would otherwise be eligible for at least low level residential care who would like to remain at home with appropriate support.
Extended Aged Care at Home (EACH and EACH Dementia) packages
EACH and EACH Dementia are packages of care tailored to meet the individual needs of older people who wish to remain living at home, but who otherwise would need nursing home care. It is a Commonwealth Government funded program, which is able to provide, arrange and purchase services on behalf of clients.
