The Hospital Admission Risk Program (HARP) - Partnerships in Health (PIH) is a service for people with chronic and complex medical conditions who frequently present or are at risk of presenting to hospital and require intensive service coordination.
The program aims to ensure clients who frequently present or are at risk of presenting to the Royal Melbourne Hospital and require intensive service coordination, are having their health care needs met in the community. It is designed to ensure people receive the right care in the right place at the right time.

Program objectives
HARP provides both disease specific (Cardiovascular, Chronic Respiratory, Complex Diabetes) and non-disease specific (Medication Management , Care Facilitation, Falls Prevention) services. The key objectives of the HARP PIH are:
to improve patient outcomes;
provide integrated seamless care within and across hospital and community sectors;
to reduce avoidable hospital admissions and Emergency Department (ED) presentations; and
to ensure equitable access to health care. |
